- Complete Application form properly notarized by a Commissioner of Oaths, Justice of the Peace, Notary Public or a Lawyer.
- Copy of diploma from graduating school and/or Qualifying Program.
- Copy of the National Dental Examining Board of Canada Certificate.
- Provide Proof of CPR - Level C - AED - valid two years from the date of issuance.
- A Certificate of Good Standing Form (valid within 3 months) from all jurisdictions where the applicant held a license or held a registration.
- Two letters of character reference from those other than family.
- Proof of Canadian Citizenship or Permanent Resident Status.
- Provide proof of Malpractice insurance currently the Board requires $3 million
- Registration Fee - $250.00 plus the Licensing Fee - The Full Licensing Fee is $1,200.00 pro rated by quarter.
- New Graduates will only pay the Registration Fee of $250.00 for initial licensure.
- Membership in the Newfoundland & Labrador Dental Association, is a requirement for licensure however there is a one-month grace period following registration with the NLDB.
