REQUIREMENTS FOR REGISTRATION AND LICENSURE
FOR SPECIALIST'S CERTIFICATION
Both the Applications for General Licensure and Specialist's Certification must be completed and submitted together.
  • Complete the Applications for General Licensure and the Specialist's Certification and have properly notarized by a Commissioner of Oaths, Justice of the Peace, Notary Public or a Lawyer.
  • Copy of diploma from graduating school and/or Qualifying Program.
  • Copy of the National Dental Examining Board of Canada Certificate.
  • Provide Proof of CPR - Level C - AED - valid two years from the date of issuance.
  • A copy of your National Specialty Examining Board of Canada Certificate.
  • A Certificate of Good Standing Form (valid within 3 months) from all jurisdictions where the applicant held a license.
  • Two letters of character reference from those other than family.
  • Proof of Canadian Citizenship or Permanent Resident Status.
  • Provide proof of Malpractice insurance; currently the Board requires that you carry at least $1 million. Contact CDSPI.
  • New Graduates will only pay the Registration Fee of $250.00 for initial licensure.
  • Licensing Fee for Specialists is $1,333.00 per full year; however, this fee may be prorated by quarter according to period of time requested, plus $250.00 Registration Fee
  • You must become a Member of the Newfoundland & Labrador Dental Association, however there is a one-month grace period following registration with the NLDB. Please contact the NLDA at www.nlda.net for their fees and information.
