PLEASE NOTE: APPLICATIONS FOR LICENSURE WITH ALL THE REQUIREMENTS ATTACHED MUST BE FORWARDED BY MAIL ONLY.  WE DO NOT ACCEPT APPLICATIONS SENT BY EMAIL. APPLICATIONS WILL ONLY BE REVIEWED WHEN ALL SUPPORTING DOCUMENTS HAVE BEEN SENT WITH THE APPLICATION FORM.   WE WILL ACCEPT CERTIFICATES OF STANDING FROM OTHER JURISDICTIONS BY EMAIL. 

  • Complete Application form properly notarized by a Commissioner of Oaths, Justice of the Peace, Notary Public or a Lawyer.
  • Copy of diploma from graduating school and/or Qualifying Program.
  • Copy of the National Dental Examining Board of Canada Certificate. 
  • Provide proof that is satisfactory to the Board that the Applicant has engaged in the practice of dentistry for a minimum of 500 hours in the 3 years preceding the Applicant's application or otherwise satisfy the board of their ability to engage in the practice of dentistry. 
  • Provide Proof of CPR - Level C - AED - (or higher) valid two years from the date of issuance .Only in-person training is accepted.  On-line training is not accepted.
  • Appplicants who are or have been licensed in another Canadian jurisdiction or territory or any international jurisdiction must arrange to have a  Certificate of Standing Form submitted directly from the relevant regulatory authority to this Board as part of the process. (Certificates of Standing are only valid for 3 months).
  • Two letters of character reference from those other than family members and current within 1 year. Letters must be signed and dated.
  • Proof of Canadian Citizenship or Permanent Resident Status.
  • Provide proof of Malpractice insurance currently the Board requires $3 million.
  • Registration Fee - $250.00 plus the Licensing Fee - The Full Licensing Fee is $1,300.00 pro rated by quarter.
  • New Graduates will only pay the Registration Fee of $250.00 for initial licensure.
  • Membership in the Newfoundland & Labrador Dental Association, is a requirement for licensure however there is a one-month grace period following registration with the NLDB.
